Tools4U is a free app for patients of Nationwide Children’s Hospital. This app provides access to clinical modules tailored towards specific patient clinical needs. The user can identify their care team and access their contact information. The two modules currently available are Complex Care and Diabetes Care. Each module looks to enhance patient education and self-management by providing the following tools:
Complex Care
• Feeding regimen and weight tracking journals
• Feeding tube troubleshooting guide
• Feeding tube quizzes
Diabetes Care
• Electronic insulin dose calculator
• Electronic blood glucose log
• “Sick Day Care” guidelines
• Carb counting resource and food records
The app currently requires a specific invitation by your care team to utilize these programs. User feedback is greatly encouraged as this will allow future updates to continually meet the needs of patients and families.
